mod_compress, problems with compress.cache-dir

Description

I'm running 1.5 r1605.

When using mod_compress without a cache-dir lightty correctly compresses and sends content to clients. However, if I set a cache-dir for it, it creates the cache files properly and sets content-encoding headers to gzip, but the content it sends is not compressed but rather the original static file.

- added better tracing with "log-request-handling"
- fixed duplicate content (plain + compressed) if cache-dir is used
(fixes #1046)
