Description

I'm adding a patch which allows the headers to be used by mod_extforward to be specified in config. For example for use with a Zeus ZXTM loadbalancer:

extforward.headers = ("X-Cluster-Client-Ip")

I suspect this patch may clash with some changes done for Lighttpd 1.4.19, but I'm not sure.

1.4.18 patch has been running in production for a while, works fine.
1.4.19 patch has not been tested yet!
