would it be possible to implement download slots into mod_secdownload?

possible? yes. do you have better description of what you seek and how you would like it to work?

might be simpler to have a FastCGI backend with a limited number of simultaneous connections, or a backend which keeps track of the number of downloads in progress for a specific URL or more general path, and implements limits there.

mod_secdownload is no longer part of the core lighttpd distribution.
